As an Assistant Practitioner in Hepatology you will be part of a brilliant, friendly, hard-working team who strive for excellent patient centred care.
The job role is primarily to provide support to the Hepatology Advanced Clinical Practitioners [ACP] and patients' who have hepatological related health problems.
The Assistant Practitioner will be responsible for administrative and clinical skills including fibroscanning.
You will be supported with on the job training opportunities to build on your current scope of practice. Providing support over the telephone to patients will be key in this position, therefore good communication skills are essential. The trust has guidelines and policies to follow that you will become familiar with to help support your role.
This is a development role, : if the succesful applicant is appointed at band 3 they will be supported to complete the Assistant Practitioner qualification as part of their development in role and once completed would be moved up to a band 4, The Assistant Practitioner will provide support to the Hepatology Advanced Clinical Practitioners [ACP] and patients' who have hepatological related health problems.
The post holder will assist with the assessment and management of patients and they will undertake a range of administrative and practical skills including independent fibroscanning, to an expert level.
The successful applicant will form a professional judgement of an individual's health condition and identify appropriate investigations to be carried out according to the correct protocols.
The post holder will manage their own role with guidance available from the ACP, supervision and teaching of staff will also be a part of this position as well as liaising with the multi-disciplinary team., 1. Deliver clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the assistant practitioners remit.
2. Organise and prioritise time effectively.
3. Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner), and deliver high standards of care in line with the plan of care.
4. Implement and evaluate individual care/treatment plans. Ensure care is documented accurately and in a timely manner as per Trust policy.
5. Support the safe admission, transfer and discharge of patients in accordance with Trust policies, under the supervision of the Specialist Nurse.
6. Provide high quality patient care, appropriate to patient need.
7. Take responsibility of care for a group of patients under the supervision of the Specialist Nurse.
8. Undertake nutritional assessment and ensure the appropriate nutritional intake of all patients considering any specific dietary needs and refer to the dietician appropriately.
9. Take personal responsibility for ensuring that patients are treated with courtesy at all times, the individual needs of the patient are respected and confidentiality is maintained at all times.
10. Promote and value the rights, responsibilities and diversity of people.
11. Work effectively as part of a multidisciplinary team recognising the contribution of all team members.
12. Actively participate in team development and make suggestions to the Specialist Nurse on service improvements.
