Assistant Practitioner Nursing

We are looking for an ambitious, dynamic, motivated, compassionate and enthusiastic Band 4 Assistant Practitioner to join our Community Nursing team in Bedfordshire. This substantive post is for the Mid Beds Community Nursing Team.

We are passionate about patient centered care and support our team to lead and deliver excellent and compassionate care., You will need to be a motivated and enthusiastic person, who has excellent communication, IT literate and problem solving skills, alongside a passion for patient centered care. You will work corporately within our Primary Care Team and will be supporting the Community Nurses and the wider MDT, to provide high quality care to housebound residents who live with a wide range of complex health conditions.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important., The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

Our Community
We provide a wide range of community health and mental health inpatient services to children, young people, adults of working age, and older adults in The City of London, Hackney, Newham, Tower Hamlets, Bedfordshire and Luton. We also provide psychological therapy services to the London Borough of Richmond, and provide Forensic services to the whole of North East London. We provide services in urban and rural settings, sometimes in our inpatient units, but mostly in the community close to where people live. Integrated care is key in making the best use of resources and providing effective care.
