We are currently developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning and frequently receive excellent feedback from our patients as well as our professional colleagues. We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience from acute and community backgrounds would be preferred, but is not essential.
