We work closely with our community colleagues and have established ourselves as a link between acute and community care in order to provide seamless transition of care between hospital and home or other receiving destination. We are currently developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning and frequently receive excellent feedback from our patients as well as our professional colleagues. We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience from acute and community backgrounds would be preferred, but is not essential. Training and support will be given to the successful applicant, including a robust induction period tailored to your needs. We are a friendly, flexible and

The team covers 7 days a week between the working hours of 8am to 8pm.
