Full Job Description

+ To undertake the delivery of nursing care as delegated by the DN team manager/Team Leader, for example, clinical skills such as catheterisation, carried out in patient's homes.
+ To work at all times in accordance with all Trust policies, procedures and protocols specific to the service.
+ To provide high quality, sensitive and competent care for patients, by demonstrating practical and ongoing application of your underpinning knowledge, as appropriate to your AP qualification. For example, decision on wound care, treatments or whether to catheterise a patient.
+ To maintain accurate patient care records in accordance with the Trust Guidelines for Records and Record Keeping.
+ To be involved in aspects of progressive information technology, i.e. inputting data for system one and communication tools as appropriate practice, within the District Nursing Service.
+ To contribute to the administration duties required by the service. For example, telephone enquiries, filing patient's notes.
+ To ensure information is accurate and up to date for future reference and complies with Data Protection Act.
Responsibility for Health & Safety
Compliance with the Health & Safety at Work Act 1974 - the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Recruitment Information for Candidates

We are seeking to appoint an enthusiastic, motivated, forward-thinking Band 4 Assistant Practitioner to work within the District Nursing service working within a team, providing home-based care led by District Nurses. The role is part time (24 hours) working across a 7-day week on the Out of Hours team, which is based at Claire House in Ince. All shifts are during unsocial hours (nights).
You must be able to travel independently across the Trust during unsocial hours, making multiple domiciliary visits whilst transporting all required equipment.
NHS Community experience is essential., Experience of working in the community is essential.
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams, with focus on client need.
An Enhanced DBS with Adults Barred List check will be obtained for the successful candidate during pre-employment checks.
