Full Job Description

Working in a busy and dynamic Perinatal Community Mental Health Team means every day will be different. You will be involved in:

  • Undertaking face to face visits in the community, including home visits.
  • Undertaking physical health checks in the community and clinic setting.
  • Working with service users and their families/carers to create holistic goal setting plans, engage in treatment planning and working in collaboration with internal/external stakeholders.
  • Managing a (capped) caseload for those under team, working closely with a Specialist Multi-Disciplinary Team.
  • Undertaking initial urgent/routine visits on behalf of the service
  • Attending Multi-Disciplinary Meetings, both internally and externally.
  • Undertaking face to face visits in the community setting including home visits
  • Main duties of the job Whilst adult mental health experience is ideal, if you have experience with working with service users with moderate to severe mental ill health and their families with a desire to work in Specialist Community Perinatal Services then we would be pleased to hear from you. As an Assistant Practitioner within the East and Waveney team, you will be working in a supportive team, with a range of professional backgrounds and experience to deliver excellent care to the families we work with. You will have access to both regular clinical and line management supervision (every 4 weeks). We also offer protected team formulation and reflective practice every month. The Community Perinatal Team are an investor in people. We believe that staff should have access to good, relevant training that helps them offer the best service to the families that use our service. There are excellent training opportunities available throughout the year with this service. This is a busy team that covers the whole of Norfolk and Waveney. You will need to hold a drivers licence and have access to a vehicle.

    You will require excellent communication and organisational skills and the ability to work flexibly.
