Full Job Description

An exciting opportunity to work alongside a multi-disciplinary team within the virtual ward. To provide assessment and treatment intervention to people transferred from secondary care with an acute illness. The aim of the service is to facilitate the early discharge from hospital to care for people in their own homes. This post is working part time (30 hours per week). The role entails assessment, monitoring and review of patients with both medical and rehabilitation needs within the 7 day 8am-8pm service with the support of the nursing and therapy staff. The base will be at Mary Potter Health Centre but patients will be seen in a variety of community settings.Main duties of the job

  • To work alongside Nurses, Physiotherapists, Occupational therapists and Assistant Practitioners within the virtual ward.
  • To provide assessment, undertaking clinical observations and treatment intervention to people transferred from secondary care with an acute illness to the virtual ward within the Nottingham City. Facilitating the early discharge from hospital and caring for people in their own homes.
  • To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.
  • To maintain records as an autonomous practitioner working with supervision
