Full Job Description

An exciting opportunity has arisen for a Band 4 Assistant Therapy Practitioner to join our developing Community Admission Avoidance Therapy Team (CAATT).
This service is based at the 'front door' of hospitals across ABUHB and provides a rapid response to people (adults) presenting with conditions that have impacted their ability to live in their usual manner with the aim of using a place-based approach to encourage and facilitate care closer to home.
The post holder will provide support to people presenting to the acute medical unit in Ysbyty Ystrad Fawr and support the ABUHB Clinical Futures strategy aimed at preventing unnecessary admissions to hospital, reducing length of hospital stay, providing care closer to home and establishing home-based support where appropriate by liaising with primary, community and third sector organisations., This role is recognised as a key part of the multi professional service delivery and we are looking for skilled, organised and enthusiastic individuals to join us. Patient management will include responsibility for the delivery of therapeutic interventions as delegated by the registered therapists along with close liaison with patients, family, members of the MDT and external agencies. Post holders will need to demonstrate good time management, prioritisation skills and flexibility in their approach as well as the ability to communicate effectively both verbally and in writing.
Successful candidates will work alongside registered Occupational Therapists and Physiotherapists and in order to provide the highest quality, most efficient service post holders will utilise competencies gained to provide a service covering the professions of Dietetics, Occupational Therapy, Physiotherapy and Speech and Language Therapy. As an Assistant Therapy Practitioner the successful candidate will manage a delegated caseload of patients, supporting their assessment and intervention plans within the bounds of the practitioner role. This will frequently including those presenting with multiple co-morbidities, frailty and cognitive deficits., The post will be based in Ysbyty Ystrad Fawr although there may be occasions when successful candidates may work across other sites within ABUHB to ensure that the service remains responsive to demands.
