Full Job Description

Ensure that the Long Term condition and planned care transformation team contributes directly to the implementation of the ICS Clinical Strategy. Lead and support the ICB LTC and planned care clinical transformation teams. Work closely with the performance team to ensure that clinical transformation resources are targeted at areas of greatest need for our system, as well as national priorities. Work closely with public health colleagues to ensure that public and population health data are used to inform the planning transformation and prioritisation of LTC and planned care services. Work closely with the ICB commissioning team, to ensure that services are commissioned in line with that data and the evidence base, to support the transformation plan and input effectively into place-based plans as required. Lead the development, implementation, delivery and monitoring of the ICS LTC, Planned Care and Cancer Programmes and associated work streams. Responsible for strategic transformation and
clinical change to support delivery of cancer waiting times constitutional targets Work closely with the population health management and health inequalities teams to support prevention and reduce health inequalities in particular with respect to the CVD prevent and cancer prevention agendas. Measure, monitor and report on programme delivery regularly through the Scheduled Care Board and to the Executive Medical Director, and other key stakeholders. Promote and enhance clinical and care professional leadership through engaging front line clinicians in co-designing future service provision. Lead projects on behalf of the ICS to facilitate the transformation of LTC, cancer and other planned care, throughout Norfolk & Waveney where appropriate for example womens health and ageing well. Working closely with the Deputy Medical Director, support the ICS clinical and care professional leads in driving improvements along entire clinical pathways. Ensure the securing of value for money, giving due
consideration to all relevant factors including risk, quality and other factors. Ensure appropriate workforce planning and development is built into the design of care models and pathways. Ensure pro-active implementation of effective people management practices and procedures and development opportunities. Analyse highly complex information, including performance metrics, financial information, data on local health needs and national/local policies, guidance and legislation to inform key transformation initiatives. Explore new ways of developing services, improving quality, managing and controlling risk whilst proactively encouraging innovative approaches and new ways of working in across patient pathways. Work with clinical colleagues and governance colleagues to ensure that clinical commissioning policies relating to long term condition management, planned care and cancer are effective and equitable, and are applied fairly across the population. Support the Chair, Chief Executive and
Executive Medical Director of Norfolk & Waveney Integrated Care Board (ICB) and other senior colleagues in responding to requests for ad hoc briefings and external engagement activities. Member of the Silver tactical on call rota for Norfolk & Waveney ICB
