Full Job Description
The post holder will be responsible for: -
o To support adults with Neurodiverse needs and their families/carers by supporting qualified clinicians to review and triage Autism referrals and log these on the patient's electronic health record.
o To complete clinic-based observations as requested by a clinician or in conjunction with a clinician as part of an adult's diagnostic assessment for Autism.
o To review and collate relevant information and map this onto a template to support clinicians as part of an adult's diagnostic assessment for Autism.
o To assist with diagnostic testing as requested by a clinician as part of an adult diagnostic assessment for Autism.
o To support and operate the referral systems and triage inputting onto Lorenzo and other clinical systems.
o To offer input into the wider adults Neurodiversity MDT meeting
o To undertake specific skilled support work and/or clinical tasks with minimum supervision as defined by Job Plan
o To undertake general enquiry calls from adults, families/carers and professionals and provide advice as required.
o Be responsible for ensuring clinical outcome data is routinely entered and appropriately collated.
o A job plan will provide further detail on the plans to develop specific skills and provide further detail on specific responsibilities.
We are currently seeking to recruit 2 motivated and enthusiastic Band 4 Associate Practitioners, who have the relevant experience and knowledge of working with adults with Neurodiverse needs and their families, to join our well-established adult autism diagnostic service.
