To work within the Community Nursing Team to provide care and support for patients in the community. The registered Nursing Associate will work as part of the Nursing workforce supporting Registered Nurses to deliver high quality, person centred, compassionate care to patients in their homes or care homes, as delegated by the registered nurse or registered health care professional. They will be part of the multi-disciplinary/integrated team delivering planned care to patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with HRCH policies and protocols, with access to advice and supervision as required.

To carry out clinical duties and be prepared to undertake clinical skills update as per needs of the service and specific patient after training and competence assessment, with support from an identified mentor for example, blood pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, wound dressings, and non-complex assessments and reviews

This will involve working independently but as part of the team covering geographical areas inHounslow., We are looking for an enthusiastic Registered Nurse Associate to provide safe and effective care as a valuedmember of the community nursing serviceas delegated by the registered nurse. The Registered Nursing Associate will work autonomously within the community using their enhanced clinical skills.
