Full Job Description

To collaborate with Speech and Language Therapists to manage caseloads working with direction and guidance from SLTs to provide a service to clients within a community setting providing advice and treatment to clients with communication and / or swallowing difficulties where appropriate.
To work as an assistant practitioner/ Care Co-ordinator to implement Speech and Language Therapy programmes devised in conjunction with and monitored by a Speech and Language Therapist for groups and individuals within the protocols, objectives and procedures of the team.
To manage own caseload:
Ensure smooth communication between therapists, families, and other healthcare professionals and facilitate the treatment journey by supporting/signposting patients/carers in all areas of their health needs, for example support with booking transport/liaising with other health professionals
Coordinate and schedule therapy sessions for clients across East Sussex, including planning and running group activities meeting individual and group needs
To use additional communication methods e.g. symbols and communication aids where appropriate
Maintain accurate client records and monitor progress, feeding back to Speech and Language Therapy Team as required
To design and create resources and low-tech communication aids for the department, the speech and language therapists, and individual client sessions, carers and other professionals, + The healthcare provider of first choice for the people of East Sussex.

An exciting opportunity has arisen to join the Speech and Language Therapy team as a Care Co-ordinator based at Centenary House, Eastbourne/Bexhill Hospital. In this rewarding role, you'll be at the heart of coordinating care, supporting clients, families, and our therapy team to ensure seamless communication and delivery of services. We are looking for an enthusiastic and motivated individual to be part of a dynamic acute and community adult Speech and Language service. This is a new role and therefore requires a flexible approach with an ability to accept an evolving remit within the Speech and Language Therapy service.

Experience in healthcare, therapy, or customer service is a plus
