Full Job Description

To work as a proactive member of the team providing a high quality, efficient and effective service to patients as part of the 2-hour response to patients at risk of admission to hospital.

To work autonomously, assuming responsibility for delegated aspects of healthcare delivery, working within a scope of practice under the direction and supervision of a registered practitioner. To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

Working as a trusted assessor undertaking initial assessments and functional assessments as delegated by registered staff and supporting clinical staff with treatment and rehab input. Assess and set up rehabilitation, support, and treatment programmes. Work within scope of competence.

Complete complex manual handling under guidance from registered professionals.

Identify and signpost for ongoing treatment and preventative input.

1. To assess, review and treat patients according to clinical need

2. To assist with activities of daily living as required

3. Complex moving and handling under guidance from registered professionals

4. Assess transfers, mobility, stairs

5. Equipment assessment and provision

6. Clinical monitoring of a patient's condition

7. Escalation and handover to registered clinicians

8. Risk assessments both in terms of direct patient safety and environment

9. Clinical falls risk assessments

10. Signposting to ongoing services

11. Provide patient focused care, support, education rehabilitation to patients to support the delivery of an agreed plan of care. Promote patient independence. Support the patients to achieve their individualised goals

12. Undertake initial assessments of new patients as delegated by the registered staff includingearly identification of risks and development of appropriate care plans

13. Provide the range of clinical support as required within Hospital at Home team

14. Following training, be responsible for a clinical case load in consultation with the relevant professionals and following codes of practice and professional guidelines
