Yes: Then the Homerton Hospital's Adult Cardiorespiratory Enhanced and Responsive Service (ACERs) may just have the answer!
We currently have a position for either a Band 6 Nurse or Physiotherapist to work within ACERS. This is an exciting opportunity for enthusiastic clinicians to join the 7 day per week, the ACERS Respiratory Team based within City and Hackney. The award winning ACERs Service are a team of dedicated healthcare professionals providing care for patients with chronic cardiorespiratory conditions within the community and hospital. This role is within the respiratory team, supporting patients with chronic respiratory conditions in their home environment., Your role will provide support patients with conditions like COPD, Asthma, Bronchiectasis and Interstitial Lung Diseases (ILD) within the community and provide a rapid response service to prevent unnecessary hospital admissions or early supportive discharge from hospital for patients with acute exacerbations of their chronic respiratory disease.
The post holders will work as part of the ACERS Acute Respiratory Infections Virtual Ward, using their specialist skills to improve patient outcomes within the home environment. The role will include working with local GP practices to provide education and provide a case management approach in a supported environment., The person specification below is not the full person specification but outlines the criteria against which your application form will be assessed.
