Asthma Clinical Nurse Specialist

An exciting opportunity has arisen for a 12 months fixed term with the Asthma Clinical Nurse Specialist Team.

Duties will be to manage own defined caseload of patients within the agreed scope of practice and taking responsibility for continuity of care acting as an independent practitioner. This will include assessing patients often with highly complex or chronic conditions, providing specialist advice across professional boundaries and planning and implementation of care, running nurse led clinics and comprehensive follow up services; and ensuring a smooth transition for patients through the relevant pathway.,

• Manage a caseload of patients undertaking physical assessment of the patient and will assume responsibility for requesting surveillance investigations relevant to the specialism (list areas of competence required), evaluating results and instigating any necessary going action.


• The post holder will maintain seamless lines of communication with the patient, as well as between secondary and primary care.The post holder will oversee management of resources ensure appropriate use of resources to promote effective patient care and the achievement of national standards.


[All clinic work will be delivered under the indirect supervision of a named Consultant).
