Full Job Description

To act as the Administrator for the HomeFirst initiative, liaising with and supporting teams within Surrey and Borders Partnership and our partner agencies who refer into HomeFirst.

Suitable for someone who is able to co-ordinate work effectively, organise and manage large amounts of data and provide support to the HomeFirst Lead in efficient running of the SharedCare Forums.

Suitable for someone who is passionate about clear, accurate recording, efficient time management and organisation; and playing an integral part in the development and co-ordination of an innovative new approach that will benefit the individuals we serve.

Main duties of the job

To be a nominated champion user for the SystmOne clinical record systems

To conduct administrator system audits for HomeFirst and provide data as required.

Take and transcribe a wide range of meeting minutes in an accurate and timely manner

Receive and process referrals to the relevant clinicians, agencies and SystmOne

Update case notes on patient record SystmOne as required

Provide secretarial support to team and SystmOne, as required, i.e., onboarding new staff - arranging meetings, ESR approvals, diary management, liaising with other agencies on behalf of HomeFirst Lead, managing SharedCare Forum calendar and co-ordinating agencies' attendance at relevant meetings, logging incidents onto DATIX etc.

Requesting data from partner agencies on behalf of HomeFirst Lead

Extract data from data warehouses i.e., DART, BI reporting

Type up reports incorporating graphics and tables as required

Type all letters dictated or provided by the team and partner agencies

Book appointments on for Shared Care Forums with relevant professionals and people referred to the service.

Answer telephone calls and relay any messages in a timely manner

Creating bespoke documents for the team, such as leaflets etc - in partnership with Comms and HomeFirst Lead

Monitor data quality of administrative input
