B5 / 6 Community Mental Health Nurse (Dev Role) mutiple roles

Community Mental Health Recovery Service Reigate is seeking enthusiastic community mental health nurses (NMC) to join the team. To work within a community team based within area working in partnership with statutory and voluntary sector to facilitate whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to patients who require the skills of a Mental Health professional, and provide a holistic, clinical autonomous nursing service, to enable individuals to engage in the best standards of care available. This will be a developmental post, progressing to band 6 after 12 months, subject to evidence of competency, extending to 24 months to achieve. The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role. Student nurses waiting for the registration will also be considered.

• defined caseload that reflects the priorities of the local population and carry responsibility for the assessment of care needs; under the supervision of the CPN Clinical Lead.
• Participate in the multi-disciplinary assessment of individuals with mental health problems living in the community.
• Ensure that a risk assessment is completed for service users.
• To be aware of the potential for vulnerability to abuse amongst the service users/families, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children and SABP Policies
• Ensure that a carer’s assessment is offered to carers of service users.
• Utilise the full range of mental health nursing skills to participate in the assessment, care and support of service users.
• Participate in the provision of community mental health nursing advice and consultation to colleagues, community groups and organisations.
• Promote the recovery model to the people who use the service and their carers
• To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.
