Full Job Description
An exciting opportunity has arisen at SHC which provide therapy services within Epsom and St Helier Hospital and the community in Sutton.
SHC -At Home service works closely with Acute services at St Helier to provide a detailed service to mainly Sutton and Merton residents with rehabilitation needs.
This post is currently under Sutton Health and Care mainly to cover community service but as a part of the team you will also get chance to spend time across various pathways within the team to enhance your learning :- elderly rehabilitation, Acute medical unit, AnE/front door, Acute Frailty ward, home-based rehabilitation.
The post holder must participate in rostered weekend work within the team roster.
To provide a high standard physiotherapy service within the service
To perform physiotherapeutic assessment of patients with diverse presentations of physical and psychological conditions, to provide a diagnosis and develop and deliver an individualised treatment programme.
To hold responsibility for own case load and be responsible for a defined area of the service, or a particular patient type, working with and without direct supervision.
Supervision takes the form of regular formal training, clinical reasoning sessions, peer review and case conferences.
Access to advice and support from a senior physiotherapist is available if required, clinical work should be routinely evaluated.
To undertake all aspects of clinical duties as an autonomous practitioner.
To liaise effectively with other disciplines.
To undertake evidence-based audit and research projects to further own and teams' clinical practice within each rotation. Make recommendations to clinical lead/manager of service for changes to practice by the team. May assist the implementation of specific changes to practice or contribute to service protocols.
To undertake training/study to ensure competency and emergency duties as a lone practitioner within the trust.
