Full Job Description
The Memory Service offers an exciting opportunity to work with older adults who are experiencing memory difficulties. The post holder will work closely with the service user and their supporters/next of kin/carers to ensure that their experience from assessment, through to diagnosis and post diagnostic planning, ensuring that their care is personalised with them at the centre. If you have an enthusiastic and positive approach to working with people with dementia and like to work in a supportive multi disciplinary team this may be your opportunity., The post holder will work within a multi-disciplinary service
- To be responsible and accountable for the high-quality standards within the service area
- To act as a positive and professional role model for junior staff and other colleagues at all times
- To assess, plan, implement, evaluate treatment and collaborative care planning
- To provide cognitive testing and assessment in the community or hub base
- To support the service manager to organize competing and conflicting priorities and manage change.
- The post holder will be the resource of clinical knowledge and competence in the service area contributing to staff and service development.
- The post holder will maintain and promote a person-centred care approach in all aspects of service delivery to service users and their significant others using our service.
