Full Job Description

The Community Stroke Rehab Team provides stroke specialist rehabilitation for Enfield residents aged 18+ with an NCL GP. Care is provided in people's home and local communities dependent on their functional goals. It is a friendly multidisciplinary team of occupational therapists, physiotherapists, speech and language therapists and neuropsychology. We have generic rehabilitation assistants to support people to carry out treatment plans set by the therapists. We work closely with adult community therapies who are also located within St Michaels.

We offer in-service team training, effective supervision and external training opportunities through the Trust.We are offering an exciting opportunity for a highly motivated Speech and Language Therapist who wishes to develop a their stroke specialist skills within community services.

The services are located near Enfield Town, with easy commuter links from central London/Hertfordshire and proximity to M25.

A car driver is essential for this role.

Main duties of the job

To undertake a comprehensive assessment of patients presenting with acquired communication and swallowing difficulties, including those with diverse or complex presentations/multi pathologies following stroke: using a high level of clinical reasoning and assessment techniques to provide an accurate clinical diagnosis.

Where the service is available, to identify and refer patients for video fluoroscopy and other objective swallowing assessment techniques.

To utilise an extensive knowledge of evidence-based practice and treatment options to select and deliver a specialised speech & language therapy programme to individual patients and plan their successful discharge.

To take responsibility for a delegated caseload, ensuring that the MDT is fully involved as required

To ensure that patients and carers are involved in the planning of their care and rehabilitation, using negotiation skills as appropriate

To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.

To provide education and training relevant to communication and dysphagia rehabilitation to MDT colleagues, patients , carers and families, as well as support community care workers with supporting these needs.
