Full Job Description

Duties for the Community Clinical Support Worker will be dependent upon competency and additional training undertaken but may include:
+ Provide delegated clinical procedures, dependent on training such as, but not limited to venepuncture, wound care, insertion/removal of urinary catheter and catheter care.
+ Administration of subcutaneous or topical medication (after additional training with competency checks)
+ To undertake patients' vital signs, blood pressure, pulse, pulse oximetry, height, weight, neurological assessment, respirations, urinalysis, blood glucose and escalate concerns in line with policy
+ Recognition of deteriorating patient and escalation process
+ Safeguard the well-being and health of patients by reporting concerns
+ Documentation of care actions and findings using the electronic patient record.
+ If required, undertaking personal care duties, such as toileting, bathing and dressing /undressing.
+ If required, assist with eating and drinking in line with care plan from Registered Professional.
This is a community post and therefore you must be able to travel throughout the locality, in a timely manner, carrying equipment/staff with you. You must have access to a car and hold a current driving license. You will also be expected to support students as part of their basic training and colleagues as part of their ongoing professional development.

It is essential that you have GCSE English and Maths grade C or 4 or Functional skills for English/Maths. You should have (or be willing to work towards) a care certificate along with relevant health care experience.
You must also have excellent team working and communication skills and the desire to share knowledge and work within an inter-professional framework.
You will be required to work flexibly which will include weekends, evenings and bank holidays.
