Full Job Description

The post-holder will provide a comprehensive Administrative Service for the Complex Discharge Team based within our Complex Discharge Hubs.

All activities will underpin the safe and timely discharge of patients. As well as work to reduce patient length of stay. The role involves cross site working on a 7 day rota to include Bank Holidays. Hours 8am - 6pm 4 days a week.

Main duties of the job

- To deal with complex information and ensure team members are aware of any issues.

- Knowledge and use of various Microsoft Office Software programmes including Outlook, Word, PowerPoint, Excel, and the Discharge Hub

Management System (DHMS).

- To prioritise the demands of the team, managing frequent disruptions and conflicting priorities.

- Receive and process all referrals triggered by ward staff. This can involve uploading referrals to Eclipse (Birmingham Council System) as

directed by managers. To record and monitor the above information on the Discharge Hub Management System (DHMS) and any other

appropriate systems/excel sheets so that the referral trail is auditable. The information is also used to produce trends, analysis and reports at

request

- Provide intelligence on Complex Discharge Hub performance. Work will involve interrogating, collecting/inputting and maintaining various

databases to produce spreadsheet reports for the Complex Discharge Hubs, e.g. length of stay of patients on the Complex Discharge Hub

caseload, discharge destinations, number of medically fit patients on caseload and other ad hoc reports.
