Full Job Description

West Midlands Rehabilitation Centre (WMRC) provides outpatient services for adults and children. These services include Specialist Neurological Services, Birmingham Wheelchair Services, Orthotics and Prosthetics. The WMRC Outpatient Nursing Team provide care to these patients during their outpatient stay. As a Healthcare Assistant, you would be carrying out direct care tasks in support of and supervised by a Registered Nurse. You will contribute to the optimum delivery of care for all patients attending Outpatients Clinic., To care for patients during their outpatient visit - this may include repositioning, toileting, assisting with transfers and other care tasks.
To assist other clinicians in delivery of care - this may include nurses, doctors, therapists and rehabilitation assistants.
To maintain high standards in the department - this may include, cleaning, stock management and audits., 1. Provide delivery of individualised care as directed by the Registered Nurse.
2. Assist Consultant/clinician in clinic as requested.
3. Prepare the clinic rooms and any equipment required for clinic in a timely manner.
4. Prepare and provide daily and weekly jobs/tasks for HCAs.
5. Communicate verbally and in writing; document in medical notes and RIO regarding patients' condition, noting any changes and reporting them to the registered nurse.
6. Responsible for data input of patient information into the NHS database i.e. RIO.
7. Monitor patient's pressure areas, assist the patient in repositioning, ensure pressure relieving equipment is used appropriately and where required; offer pressure relief to patients who may be delayed for clinic or transport.
8. Ensure the clinical area is clean and tidy in line with Trust Infection Prevention and Control Policy.
9. Act as Link Worker for the Trust Infection Prevention and Control Team.
10. Communicate verbally and in writing regarding patient's condition, noting any changes and reporting them to the Registered Nurse. Attend daily meetings.
11. Communicate with patients who may be upset, anxious or suffering from communication difficulties, due to changes in their physical, mental or emotional conditions.
12. Ability to deal with sensitive information and have a good understanding of Confidentiality and associated policies.
13. Carry out or assist the patients/carers with activities of daily living, such as feeding patients, toileting and washing.
14. Carry out observations of care, temperature, pulse and blood pressure; complete patient information for Essential Care Indicators (ECIs) and report all findings to the Registered Nurse.
15. Complete Patient Care Plans under the guidance and support of the Registered Nurse.
16. Provide basic wound care including re-dressing, superficial wounds under the guidance of the Registered Nurse. Attend wound care training update annually.
17. Ensure that patients with continence problems are given the correct appliances to meet their needs. Empty or change catheter bags as needed.
18. Assist with patients requiring hoisting. Follow moving and handling assessment as completed by Health Care Professional.
19. Respond appropriately in an emergency situation, e.g. cardiac arrest. Observe and report any changes of condition to the Nurse in Charge.
20. Chaperone patients as requested.
21. Assist reception with meeting and greeting patients; and liaise with reception and the clinical team for patients on transport or using interpreting service.
22. Participate in the maintenance of agreed standards of care with other members of the interdisciplinary team.
23. Awareness of the Division and Unit objectives, ensuring they are met within the team to maintain professional standards.
24. Escort patients to departments as designated by the Clinician.
25. Awareness of the Policies and Procedures that must be adhered to, to ensure effective patient care and Health and Safety is maintained.
26. Participate in Team meetings to ensure effective communication is maintained.
27. Participate in the development and maintenance of a quality service, reinforcing the key values held within the Centre.
28. Participate in training programmes and attend all mandatory training, taking responsibility for remaining updated.
29. Support new staff by showing them how to undertake the jobs required of a Healthcare Assistant.
30. Participate in health promotion programmes for patients e.g. MS awareness and brain injury.
31. Ensure that all visitors to the Centre are approached in a helpful, welcoming manner.
32. Undertake the ordering of clinical supplies, ensuring that stock levels are maintained in the Centre.
33. Participate in Audit/Research as required.
34. Act as a Mentor and Supervisor for more junior members of staff such as: work experience staff and newly appointed Healthcare Assistants.
