Full Job Description

To work as part of the Community Nursing Team and undertake assigned tasks under the supervision of registered Nurses providing effective, safe patient focused care at all times, and to work without direct supervision when escorting service users in the community. To work as a member of the Multi-disciplinary team supporting people with Learning Disabilities, mental health and physical health needs, attending clinics and Doctors reviews as required. Contribute to the development of the care environment with a positive, professional attitude. The post holder will be trained to ensure that issues of equality, diversity and patient rights are dealt with in accordance with good practice and legislation.,
