Full Job Description

The post holder will work with patients as part of a multidisciplinary team to facilitate their rehabilitation and enable them to achieve the optimum possible independence, within the parameters of the Early Intervention Community Team (EICT) The post holder will support the clinicians in delivering the rehabilitation programme following initial assessment, in the patient's own home or in a community facility. The post holder will be expected to work shifts as required over 7 days per week in line with service delivery.,
