Full Job Description

An exciting opportunity has arisen to join the HIV Team at Birmingham Heartlands Hospital in a new role of service coordinator. Working alongside the clinical team you will provide a point of contact for services users enabling them to access support services. The post holder will act in a social prescriber role to include but not limited to assisting service users with housing and benefit applications, accessing peer support and third sector agencies, completing applications for grants, accessing food banks, support into education or job applications and signposting to a range of services that may meet social requirements. The role is ideal for an aspiring social care and health professional.Main duties of the job You will be given the opportunity to use your own initiative and deal with the unpredictable. The post holder will deal professionally with enquires from staff, patients and partner agencies. You will have a thorough understanding of confidentiality in everyday working practice. You will:

  • Provide a comprehensive co-ordination service, working in conjunction with colleagues, service users and stakeholders to provide an efficient service.
  • Liaise with staff and service users and understand own limitations.
  • Accurately record all interactions to patients/service users.
  • Provide effective communication and problem solving both face to face and via telephone.
  • Be sympathetic and sensitive to the requirements of staff, service users when communicating by telephone or face to face, including dealing with difficult situations.
  • Contribute to building effective teamwork in exchanging views, ideas and communicating effectively.
  • Ability to recognise situations that should be referred onto the Line Manager and take prompt and appropriate action
  • Develop a comprehensive record of social care and voluntary services that may benefit current and future service users.
