Full Job Description

The Single Transfer of Care Team are looking for a well-organised, enthusiastic individual who can support the day-to-day operational management of the Single Transfer of Care Team within this service and undertake the role of Deputy Operations Manager. The right candidate will have the relevant experience to lead a Team, support them in managing a challenging and patient-centred workload, as well as adapting to changing priorities where the service needs require a responsive approach to ensuring patients are admitted and discharged safely and timely. Experience and knowledge of the Discharge to Assess pathways, Home First ethos and ways of working would be an advantage, as well as a willingness to be flexible across the hours of service, supporting all inpatient wards and areas as part of your working day. Applications are welcomed for full and part-time working, and if you wish to find out more about the post, or arrange an informal visit to the Hub, please don't hesitate to contact us on 0121-466/6101 Option 3., To provide key support on a day-to-day basis to the Hub Operations Manager, Group Manager and other Leads, and the Hub team members across all sites, undertaking PDRs, managing sickness, one to ones, arranging equipment and uniforms etc. Undertaking regular team meetings, sharing good practice across the Hub Team Arranging cover during times of leave, this may include stepping in where cover is not possible working where required as an active member of the team in any role. As required lead the operational day-to-day running of the Hub to ensure processes are working, be reactive and work through any issues that require escalation or prioritising. Working as senior operational support to all Leads for the Hub, to include - Day to day management and allocation of all time-driven tasks Ensuring complex cases for both admission and discharge are discussed on the Bronze, Silver and Gold system-wide calls as required, providing consise and clear Ensuring that all data sets and reports are circulated and submitted accurately and on time to the Trust and wider system partners on a daily basis Representation on all regular meetings held where the Integrated Coordination Hub is required to participate, having the confidence to present complex information across all aspects of patient flow processes and outcomes,
