Full Job Description

To work as part of the KCRS in the delivery and co-ordination of exercise based Pulmonary Rehabilitation ensuring high quality, timely and evidence-based care. To provide evidenced-based care to patients with a variety of underlying chronic respiratory disease, including but not limited to COPD, bronchiectasis, asthma and pulmonary fibrosis., + To assess, plan, treat and review the exercise needs of patients referred for Pulmonary Rehabilitation with an underlying chronic respiratory disease.
+ To deliver the exercise element and participate in the education program of Pulmonary Rehabilitation.
+ To ensure that patients progress appropriately through sessions and give advice and information as necessary.
+ To refer patients to appropriate community support programmes.
+ To communicate effectively with members of the multi-disciplinary team to ensure holistic patient care.
+ To provide PR in a variety of clinical settings, including; leisure centres, community venues, gyms, virtual platforms, home-based and utilizing exercise prescription platforms.
+ To be able to work as part of a team, but also to work independently in a variety of clinical settings.
MANAGERIAL/LEADERSHIP
+ Maintain accurate and legible records of patient care and activity in line with departmental guidelines and trust policies.
+ Assist with the organisation and delivery of Pulmonary Rehabilitation.
+ To support the development of students who maybe on placement within the service.
+ To support in the development of assistant staff within the service.
ORGANISATIONAL
+ To undertake personal development in line with trust policy.
+ To perform duties in line with the trusts Values & Behaviours.
PROFESSIONAL
+ To participate in Professional Development Review as per trust policy
+ To maintain up to date knowledge regarding evidenced based exercise in chronic respiratory disease.
QUALITY
+ To provide high quality, individualized evidence-based exercise interventions.

We expect all new starters in clinical, patient facing posts (bands 1-4) to complete all 15 standards of the Care Certificate, within their first 6 months with the Trust.

A fantastic opportunity has arisen for committed, motivated and driven individual to work as an Exercise Instructor in the Community Pulmonary Rehabilitation service. The role will include working across community venues, in patients' homes, being a core member of the multi-disciplinary team and empowering patients to make informed choices about their health and health care.
This role demands the ability to be innovative, flexible and a strong team player and be able to work autonomously. Due to the nature of the role, it is essential that applicants can travel independently across the locality and have access to transport for work purposes.
You will need to work within your own scope of practice and be able provide pulmonary and exercise rehabilitation support to patients with a Respiratory diagnosis.
The perfect candidates for our service will possess a passion and desire to deliver a high standard of care to create an outstanding patient and family experience and the leadership skills and drive to inspire.
