Full Job Description

The Joint Community Rehabilitation and Falls Prevention Service is a short-term 7 day service which assesses and treats people within their own homes. The team, uses an inter-professional model of working and aims to prevent unnecessary admissions to hospital or long-term care, enable earlier discharges from hospital and promote independence and quality of life. We key work and provide rehabilitation to adults with a variety of medical and surgical conditions. Weekends are covered on a rota basis.

Main duties of the job

To undertake holistic assessments, key work individual cases and provide specific Physiotherapy intervention as required. To manage a clinical caseload and to participate as part of an integrated team to meet individual clients rehabilitation needs

As well as offering continuing professional development and clinical supervision, we also offer excellent support for those wishing to develop a special interest as well as a comprehensive in-service training schedule.
