Full Job Description

We are excited to be recruiting to our new Acute Care Response Team which is an integral part of the Deteriorating Patient Team., The Acute Care Response Team Enhanced Practitioner will be required to:

o Have an Enhanced level of knowledge in the prevention and management of the deteriorating patient utilising the NORF Enhanced Practitioner Framework.Appendix-1-Enhanced-Critical-Care-Outreach-Competencies.pdf

o This autonomous role will exercise independent judgement to assess, investigate, plan, implement, evaluate and escalate clinical care of the deteriorating patient.

o Provide patient centred care to a high standard usually within complex and evolving clinical scenarios utilising their enhanced clinical knowledge and expertise.

o Make complex clinical decisions and work collaboratively with the multidisciplinary team to support the care of patients including discussions surrounding treatment and escalation plans and DNACPR discussions.

o Support clinical teams across the organisation including Medicine, Surgery, Maternity, Emergency Medicine and Paediatrics by providing enhanced clinical advice, demonstrating leadership and support to ensure effective and safe provision of care to the "deteriorating patient".

o Support, deliver and evaluate education, research and audit activities to elevate and improve standards associated with the ACRT service.

The role as ACRT Enhanced Practitioner is underpinned by the Critical Care Outreach Practitioner Framework. andsupports a 24/7 service across the Watford site and currently Mon-Fri Night at St Albans which aims to prevent, identify, escalate and respond to patient deterioration along with supporting successful transition of patient care from the critical care environment.The Enhanced Practitioner (Band 6/7 depending on experience) will be able to assess and treat patients in a variety of clinical scenarios.

The post holder will demonstrate enhanced skills and competence in areas such as advanced physical assessment and ensure that evidence-based practice underpins all aspects of care and treatment. They will have developed enhanced skills in critical thinking and problem-solving to support safe clinical decision making.They will deliver a high level of specialist care to patients outside of the Critical Care Department working within a defined scope of practice that is underpinned by both local and nationally agreed guidelines.

Those shortlisted will be required to attend in person an assessment day on Thursday 6th February. If successful at the assessment day candidates will be invited to interview on Monday 10th February.
