An exciting opportunity has arisen for an experienced cardiology/cardiothoracic nurse to join our Cardiac Health and Rehabilitation Programme in St Marys and Charing Cross Hospitals as part of the Imperial Healthcare Cardiovascular Health Services.

For a Bank Nurse within the Cardiac Rehabilitation service you will assess and manage out-patients and participate in the face-to-face Cardiac classes. You will act as a resource to other professionals on cardiovascular disease and associated treatments and issues. You will be taking part in ongoing audit and research activities to maintain quality care provision. You may also be required to provide occasional cover for our in-patient service at Hammersmith Hospital.

For this challenging and fulfilling post we are seeking to recruit an enthusiastic individual with sound knowledge and understanding of the prevention and management of cardiovascular disease The Cardiac Health and Rehabilitation Programme provides care to patients during the in-patient phase and to those who have been discharged and are attending the cardiac health and rehabilitation programme. It includes patients with cardiac disease, heart failure as well as those with ICDs.

Main duties of the job

To participate in the provision of information to patients and their carers regarding their disease or risk of disease, treatment and service available at all stages of their cardiovascular disease process either one to one or group sessions.

To participate in the teaching programme on cardiovascular disease risk factors for patients and relatives

To support local patient support groups, or in their absence, facilitate the development of such groups, to facilitate user involvement when producing relevant work. To continue to support the local Heart Support Group. To maintain close links with Phase IV (community) exercise instructors

Support the nursing team in the training and education needs of the multidisciplinary team and promote specific skills related to secondary prevention & rehabilitation of patients with cardiovascular disease. Work collaboratively with relevant educational staff/providers to deliver teaching programmes to meet these needs.

Updating own knowledge to promote excellence in clinical practice.
