Full Job Description

As a band 6 cardiac specialist nurse within the team you will be required to assess, plan, deliver and evaluate patient centred care including medication optimisation, symptom management, and education to aid improved quality of life. You will be working in partnership with patients, carers and other health and social care and voluntary agencies to promote health and well-being.

You will be required to work multi professionally as part of the Cardiac Service, providing care for individuals requiring management of their heart failure. You will be required to support patients within their own homes, community clinics, cardiac venues, and virtual appointments.

An exciting opportunity has arisen for a dedicated and motivated existing Cardiac Nurse to work within the Cardiac Services Heart Failure Team of Birmingham Community Healthcare NHS Foundation Trust Adults and Community Services Division. Training will be offered to meet the services requirements and ensure you are fully supported in your career development.
