Full Job Description

As a band 6 cardiac specialist nurse within the team you will be required to assess, plan, deliver and evaluate patient centred care including medication optimisation, symptom management, and education to aid improved quality of life. You will be working in partnership with patients, carers and other health and social care and voluntary agencies to promote health and well-being.
You will be required to work multi professionally as part of the Cardiac Service, providing care for individuals requiring management of their heart failure. You will be required to support patients within their own homes, community clinics, cardiac venues, and virtual appointments., + Please ensure you check your Trac account regularly as this is how we will communicate with you during the shortlisting and selection process.

An exciting opportunity has arisen for a dedicated and motivated existing Cardiac Nurse to work within the Cardiac Services Heart Failure Team of Birmingham Community Healthcare NHS Foundation Trust Adults and Community Services Division. Training will be offered to meet the services requirements and ensure you are fully supported in your career development., You will work autonomously and have the ability to work under pressure. Excellent communication skills are essential as you will need to be committed to sharing good practice, development of robust professional networks and support your colleagues. You will have good computer skills, and are able to utilise excellent time management and organisational skills. Full training will be given to the successful candidate.
The successful applicant will be required to work Monday to Friday between the hours of 8.00 am and 17:00pm. Consideration will be given to flexible working hours that meet the needs of the service.
