Full Job Description

The Community Nurse Specialist (CNS) Access Team Leader is an expert in ensuring high quality palliative care is delivered to patients and their families. They work independently, and as part of the wider health care team. To offer the patient and families the skills of a clinical nurse specialist in symptom control, social, emotional and spiritual care. Independent Prescriber Lead, develop and line manage the CNS Access team to work efficiently and effectively in the delivery of high quality and progressive palliative care in line with the Hospice strategy and policies. Offer advice, support and education to Hospice Teams, Primary and Community Healthcare staff as part of a multiprofessional team. To work using a multiprofessional approach to care. Develops collaborative working relationships with other hospice teams and the wider community services.

We are a progressive and dynamic community nurse specialist team at the forefront of delivering outstanding palliative care. Due to a recent service restructure, we are looking for a passionate and enthusiastic individual to lead the dedicated team of community nurse specialists managing our advice line. The successful candidate will primarily support patients via the advice line, but also have the ability to responsively visit patients at home when urgently required. To apply for the role you will need to be a non-medical prescriber with a minimum of 2 years specialist palliative experience. We would also welcome applications from candidates with a minimum of 18 months experience in a specialist palliative role, evidence of leadership / management and a willingness to undertake the independent prescriber module. The Community Nurse Specialist Access Team support any palliative patient, family and health care professional (within BNSSG) contacting the hospice advice line within
the hours of 07:45 to 20:00 Monday to Sunday including bank holidays. We can offer eitherfull orpart time hours (37.5 or 30 hours per week)
