An exciting opportunity has arisen for a proactive, dynamic and inspiring Band 6 Physiotherapist to join Taunton & West Somerset Community Rehab Service. Working with people on their identified rehabilitation goals to reach their maximum potential and personal health & well-being in their own homes or local communities. You will be working alongside OTs, Rehab Assistants / Practitioners and Pharmacy Tech and liaising with other health, social and voluntary services. You will be supervising other staff and participating in the Duty rota. We run an in-service training programme and continually aspire to improve the service we deliver. Main duties of the job The main aim of the Community Rehab Service is to enable patients to work towards their own identified goals in their own environments. The post holder will be required to use enhanced knowledge and skills to assess and facilitate rehabilitation programmes with patients and their support network to maximise their daily abilities in their daily life and ongoing self management skills. There will be a requirement for post holders to work closely with colleagues in the local neighbourhood and community services / teams. You will be expected to carry out specific therapy assessments and devise rehabilitation programmes. This will include treatment planning, assessing risk, promoting and enabling independent functioning. You will be committed to a person-centred approach to your work and have an ability to prioritise, Please see attached full job description.Maintains good communication, liaison and working arrangements with other Trust teams, Trusts and external agenciesTo use communication tools (verbal and non verbal) to gain valid informed consent with patients where there will often be barriers to effective communication loss of hearing, altered perception, expressive and receptive dysphasia, pain, fear. To have the capacity to work within a legal framework with patients who lack capacity to consent to treatment those with cognitive or speech difficulties.

Essential

  • Degree or equivalent in Physiotherapy
  • Desirable
  • Evidence of further relevant training qualifications.
  • Experience Essential
  • Qualified Physio experience in wide range of settings and of range of diagnosis'.
  • Desirable
  • Previous Band 6 experience
  • Previous community rehab experience.
  • Additional Criteria Essential
  • Must be a car driver with a car available for work.
