Full Job Description

We have an exciting Fixed Term or Secondment opportunity for an experienced Community Physiotherapist or Occupational Therapist who would like to extend their leadership and managerial skills whilst guiding and supporting more junior members of the team. You will have the opportunity to develop skills in education and clinical leadership within this role. We are looking for an enthusiastic, forward thinking, team player to join the Derby City Core Therapy team. Your responsibilities will include managing a wide array of clinical conditions including neurological, orthopaedic (trauma and elective), respiratory and elderly rehabilitation. You will be working as part of an integrated team with Physiotherapists, Occupational Therapists, Advanced Therapy Support Workers, Therapy Support Workers, and alongside Social Care, Nurses, Community Matrons, Care Coordinators, ACPs and other specialist services. Our Integrated Care Services Teams are enthusiastic with a breadth of experience and a shared goal to offer the highest standard of care to our patients. All team members are expected to carry their own clinical caseload, attend regular therapy meetings and undertake individual and joint therapy assessments and treatment sessions with members of the MDT where appropriate. Rehabilitation is delivered to patients in community settings with a focus on improving health and well-being, working toward patient centred goals and preventing hospital admission or facilitating discharge home. You will be able to develop your knowledge and experience around Falls and Frailty, and will also have the opportunity to work with the unplanned Therapy team to prevent hospital admission and facilitate discharge. The Core Therapy hours are between 8.30am and 4.30pm, however we support Community First which is a service 7 days a week between the hours of 8.00am and 6.30pm. Weekend and bank holiday shifts will be on a rota basis.
