Full Job Description

The post holder will participate in effective and timely transfer of patients from the acute Trust through identified social and health care pathways, to improve the coordination and delivery of care thereby reducing delays and expediting discharges.

To support the coordination of discharge arrangements for patient with complex needs as part of the National Hospital Discharge service, ensuring that the patient is at the centre of all discharge planning.

To ensure patients are discharged at their 'optimised for discharge' date in an efficient and timely manner.

To ensure that patients are discharged to their place of usual residence: otherwise, they are placed with services relevant to their needs, that opportunities of independence and self-management are maximised and that reliance on long term care is mitigated wherever possible thus reducing the likelihood for readmission.

Main duties of the job

To support the discharge planning and coordination for patients within the Trust who have complex healthcare needs utilising the Discharge to Assess Model.

To provide effective liaison between health and social care professionals and other relevant agencies to support discharge.

Adopting the home first principle when discharge planning. Assessing End of Life patients and making referrals to ensure patients are discharged to their preferred place.

Supporting the ward with complex discharges and regularly communicating to provide progress reports. Supporting patients to reduce their length of stay and creating flow throughout the Hospital.
