Full Job Description
To work as an autonomous practitioner in assessing, implementing and evaluating specialist care to children with life limiting/threatening illness, complex needs and technology dependence in a variety of community settings.
- To work as named nurse for a designated caseload of children with complex, acute and chronic health care needs across the Diana Service.
- To work closely with Diana Complex Care Clinical Lead to lead and co-ordinate continuing care packages.
- Teach, train and support Diana specialist practitioners , social care workers, parents and carers health care tasks in a variety of settings using peer assessment and LCAT.
- To act in an advisory role to other professionals and staff from other agencies in respect of health care for children and families. Providing care and support to families and carers of children with terminal, palliative and complex care needs.
- Participate in the service provision for 24-hour-on-call for staff and end of life care.
- Attend and support all essential training as specified by the Diana Service to maintain own and others clinical competence.
- Support other staff within Diana Team. Directly supervise Band 5, 4 and 3 staff within team.
- Carry out personal development planning with Band 5, 4 and 3 staff.
- Demonstrates substantial application of skills and knowledge in Children's Nursing in a variety of settings including community., To work autonomously taking responsibility for care, quality and clinical activity for a designated caseload/team
- To contribute to care quality performance and clinical activity by providing professional nursing leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality.
- Patient safety: The effectiveness of all health care interventions
- Patient experience: The environment in which care is delivered
- To participate in the effective clinical leadership and management of the team.
- To actively contribute to the improvement of service and quality of care provision to meet the needs of patients, service users and their families.
- To act as role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff.
- To work collaboratively with other senior nurses across LPT as appropriate and promote the role of their speciality nursing to all stakeholders.
- To foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders.
- Responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of patients according to changing healthcare needs and varying levels of complexity.
