Band 6 Health and Justice Vulnerability Service - Adult Outreach

As part of the selection process you may be required to undergo a Disclosure and Barring Service (DBS) check, however all cases of criminal record history are dealt with on a case by case basis.
Please note, if applicable for your role you will be required to cover the cost of your DBS certificate. This will be deducted from your salary over 3 months. You will also be required to sign up to the DBS Update Service before commencement with the Trust.
All successful applicants will be required to complete the Trust's induction programme. The Fundamental (Statutory and Mandatory) training will be in an e-learning format. You will be required to complete training modules online (approx. 9-14 hours depending on post) by the end of your first week with us.

If applicable to your role, the Trust will require you to sign up for the DBS Update Service. The subscription will cost you £13 a year but the Trust will reimburse this. You will be contractually obliged to maintain your subscription.

We are looking for experienced, versatile, motivated , qualified nurses, social workers or allied health professionals with post qualification experience to deliver a service to vulnerable individuals who are often the most marginalised within society. You will offer them hope and support in reducing their health inequalities and support with diversion away from the criminal justice system., + Be a suitably qualified professional(Nurse, Social Worker, Occupational Therapist, Speech and Language Therapist or other allied health professional)
+ Extensive post qualification experience working in a range of settings, including community work, of at least 12 months,
+ Excellent communication skills
+ Ability to work collaboratively and in partnership with agencies such as Police, Probation Services, Drug and Alcohol Services and many more!
+ To demonstrate sound assessment skills including identifying risk and informing and promoting decision making around complex care or safeguarding needs (mental health, exploitation and abuse, substance misuse, homelessness, learning disability)
+ To manage a caseload of adults referred to the service, empowering them to access the support to improve their lives.
+ To be a strong team player and able to work autonomously
+ To work in partnership with service users, families/ carers with regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.
