Full Job Description

This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; acting as gate keepers to admissions; supporting early discharges from hospital. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service. To help you achieve expanding your knowledge we will offer you
