Band 6 Home Treatment Nurse

This role involves intensive support and treatment to patients in the community in the acute phase of a mental health crisis as an alternative to inpatient care; monitoring patients' safety and well-being to promote independence in the community; acting as gate keepers to admissions; supporting early discharges from hospital. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads/Senior Practitioners and to have close working relationships with the community teams, inpatient services, and the HTT backup across the service.
