Band 6 Hospice at Home Nurse / Clinician

The Hospice at Home Team support patients who are in the last weeks of their life and want to be at home. The team works closely with families, community nursing teams and the wider hospice services to give the best care and support possible at this crucial time. Joining the Hospice in this role will involve planning and providing specialist palliative care for our patients and support for their families. The rolerequires independent working in the community; and as part of the wider health care team. You willhave excellent communication and IT skills and be able to coordinate the service, assess and support patients and families and line manage Band 5 nurses and healthcare assistants to deliver Hospice at Home care., The Hospice at Home Registered clinician is an expert in ensuring high quality palliative care is delivered to patients in the last weeks of life and their families. They can work independently, and as part of the wider health care team. The role includes Providing symptom control, psychological, personal, social and spiritual care and initial after death guidance and bereavement support Support and line management of Band 5 nurses, Nursing Associates and Healthcare Assistants The triage, assessment and prioritisation of referrals Coordinate, manage and allocate HCA teams to deliver Hospice at home care Working with a multi-professional approach to care Participation in quality initiatives
