Full Job Description

Service users are supported by a full multi-disciplinary team with the aim of enabling them to live independent and fulfilled lives. The team provides intensive quality interventions between 8am and 8pm, 7 days a week.
As a Key Worker you will be responsible for managing a small, but intensive caseload of service users and supervising a mini team of recovery workers. You will also be expected to provide leadership and coordination to the team on a day to day basis.
The successful candidate will have extensive experience of working with people in both hospital and community settings with a variety of complex needs and will be able to demonstrate a high level of skill working as an autonomous practitioner and within a team. You will also be able to demonstrate recovery in practice and positive risk taking. A professional qualification is essential with current registration.
The successful candidate will contribute to the on-going development of the service and assist the MDT in identifying new referrals to the service., The post holder will work within a multi disciplinary team to work with people to identify their own recovery goals and take back control of their own lives in order to live as independently as possible. As a Band 6 Key Worker within the team you will be responsible for the care coordination of a small number of people and the organisation and supervision of a mini team of recovery workers.
You will also be responsible for coordinating the day and responding to crises where necessary, delegating tasks to colleagues.
As a member of a continually developing service the post holder will have the opportunity to contribute to the development of the team., o Evidence the implementation of Recovery Focussed practice
o Responsible for the collaborative assessment, planning and delivery of recovery focussed goals including developing strategies in regard to positive risk taking
o Responsible for coordinating crisis management
o Ensure delivery of recovery focused interventions which include the provision of recovery based activity.
o Encourage creativity in recovery planning within the processes of the Mental Health Case Framework
o Undertake assessments of people referred to the services
o Coordinate in-reach to referring services to identify early goals and relationship building
o Develop graded approaches to discharge from hospital in conjunction with existing Multi-Disciplinary Teams
o Build positive collaborative recovery relationships within professional boundaries
o Deliver quality interventions appropriate to recovery needs using Psycho-social Interventions/Cognitive Behavioural Therapy informed practice.
o Monitor and evaluate risk through the use of tools such as the Detailed Risk Assessment and Management Plan (DRAM)
o Creation of relapse prevention plans which identify early warning signs and triggers to ensure timely and proactive response
o Complete relevant assessment tools and outcome measures including clustering
o Be actively involved in clinical review including Multi-Disciplinary Team meetings
o Complete carers assessments and support families, referring to family intervention services where appropriate
o Complete Self Directed Support assessment tools and support plans
o Follow policy and procedure to ensure safe storage of medication
o Administer and monitor medication prescribed to people providing information and advice., o Undertake mandatory and appropriate training relevant to your role
o Identify own training and developmental needs
+ Provide cascade learning of recovery focused practice and other expertise skills such as psycho social interventions
o Assist in the delivery of training in areas of personal experience and interest
o Prepare reflections of positive working or difficult situations
Supervision
+ Supervise recovery workers according to activities and duties identified within recovery plans, these could include the following
+ identify their own goals to improve their quality of life
+ write their own recovery plans
+ to develop recovery plans with their care coordinator and implement them creatively using initiative.
+ express their needs and wishes
+ emotional needs
+ locate appropriate accommodation
+ ensure that accommodation is appropriately furnished and personalised.
+ apply for benefits
+ develop skills in order to attain and maintain independence
+ activities of daily living
+ maintain their personal hygiene and physical health.
+ actively engage in health promotion
+ vocational and educational aspirations
+ engage in meaningful and enjoyable activity
+ attend appointments
+ to structure their time positively
+ initiate, maintain and strengthen familial and social relationships
+ increase self confidence
+ embrace new opportunities
+ plan future recovery goals
+ identify appropriate and timely progression from CERT
o Participate in and facilitate annual performance development reviews
o Participate in and facilitate regular individual and group supervision
o Provide immediate support and de-brief as a consequence of untoward or difficult circumstances.
+ Act as a mentor to students
Governance
+ Ensure that up to date electronic records and activity data are maintained in accordance with professional and trust standards and provide specialist reports.
+ To keep accurately and timely record of each contact on Insight
+ Complete outcome measures as required
+ Respect the individuality, values, cultural and religious diversity of people and contribute to the provision of a service sensitive to these needs.
+ Ensure performance targets and standards are met
+ Complete clinical outcome measures are completes as required.
+ Adhere to professional codes of conduct, professional standards and ethics and comply with local and national
Management and Leadership
+
o Coordinate and manage the working day dealing addressing issues as they arise
o Provide a high level of leadership to recovery workers
+ Ensure that recovery underpins every aspect of the team
+ Support the team manager in the completion of rotas
+ Manage and allocate work within mini-teams
+ Contribute to the induction and training of staff and students
+ Support the management team to monitor training/supervision undertaken
