Full Job Description

The Older Persons Assessment and Liaison Team at the Heartlands Hospital are looking for a highly enthusiastic and motivated clinical nurse specialist who has a passion for the care of older adults.

As part of a multidisciplinary team OPAL provides timely patient centred assessments to those with acute needs within the Heartlands Hospital. We provide effective and efficient multidisciplinary comprehensive geriatric assessment within the ED, OPAD and MAU.

Our aim is to reduce avoidable admissions by proactively assessing patients and addressing issues relating to function, mobility and social needs referring on to community services as required.

The post is based within the OPAD, MAU and ED and work will be undertaken within these areas. The service runs 12 hours per day, 7 days a week.

The team adopt a home first approach using client centred assessments. You will use your leadership, expertise and advice to support the patients through their journey., As part of a multidisciplinary team (MDT), OPAL provides timely patient centred assessments to those with acute needs within the Queen Elizabeth Hospital. You will provide effective and efficient multidisciplinary comprehensive geriatric assessment (CGA) within the Emergency department and the Acute Medical Unit.

We would expect applicants to have excellent communication skills, a flexible approach and the ability to work autonomously. Excellent organisational and time management skills are essential.

You will have excellent prioritisation skills and demonstrate client centred practice. You should provide evidence of relevant courses and training as well as commitment to ongoing CPD.

You should have relevant clinical experience specific to the post, and you will need to have experience of working within the acute NHS as well as working within a multi-disciplinary team.

The successful candidate will be required to supervise staff as well as support undergraduate students within their clinical placements.

You will be required to work long days (0730 - 2000) in line with your contracted hours to meet the service needs.
