Full Job Description
- Deal with triage of patient referrals
- Complete holistic initial assessments with patients
- Liaise with family members/relatives and carers
- Devise and reviewing care plans
- Complete follow up visits with patients
- Attend and participating in team meetings and in service training.
- Be part of a team that responds to 2 hrs referrals as part of the UCR ( Urgent Care Response).
This is an opportunity to put your Degree in Physiotherapy to great use. You will have a full driving license with access to a car, you will be motivating, innovative and adaptable, with the ability to work flexibly across specialities as need arises, holding the HCPC (Physiotherapist) registration. With strong communication and organisation skills, and a flexible approach you will ensure delivery of positive experiences to patients at every stage of their healthcare journey. You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, EMIS - Salary from £37,338 - £44,962 with NHS Pension and full Agenda for Change terms and conditions
