Band 6 Senior OPAL Physiotherapist

As part of a multidisciplinary team OPAL provides timely patient centred assessments to those with acute needs within the Queen Elizabeth Hospital. We provide effective and efficient multidisciplinary comprehensive geriatric assessment within ED, AMU and Frailty Same Day Emergency Care (FSDEC).

Our aim is to reduce avoidable admissions by assessing patients in relation to function, mobility and social needs referring on to community services as required. This holistic service means patients receive the support and treatment needed, in the most appropriate setting for them, which will often be their own home supported by community teams.

You will be adopting a home first approach using client centred assessments, using your leadership, expertise and advice to support the patients through their journey.

You will also support the new FSDEC which was opened by OPAL in November 2023. The area provides early CGA's for patients who are 65 and above with the aim to facilitate early discharge and reduce length of stay., As part of a multidisciplinary team (MDT), you will provide timely patient centred assessments to those with acute needs within the Queen Elizabeth Hospital. You will provide effective and efficient multidisciplinary CGA within the Emergency department, Acute Medical Unit and FSDEC.

The role requires excellent communication skills the ability to discuss highly sensitive matters with patients and relatives. You will also be able to confidently refer patients for onward support in the community and provided sign posting information., We would expect applicants to have excellent communication skills, a flexible approach and the ability to work autonomously. Excellent organisational and time management skills are essential.

You will have excellent prioritisation skills and demonstrate client centred practice. You should provide evidence of relevant courses and training as well as commitment to ongoing CPD.

You should have relevant clinical experience specific to the post, and you will need to have experience of working within the acute NHS as well as working within a multi-disciplinary team.

The successful candidate will be required to supervise staff as well as support undergraduate students within their clinical placements.

The ideal candidate would be passionate about elderly care within an acute setting.
