An exciting opportunity has arisen for a full-time, permanent CHC Nurse Assessor within the Adult Specialist Rehabilitation (ASR) division. The post-holder will provide clinical input in the assessment and case management for CHC and will ensure that work programmes are planned, managed effectively and successfully delivered. Responsible for facilitating assessments and reviews against the national framework, within the agreed timescales, the successful candidate will promote high standards of communication and liaison with patients, their representatives and Health and Social Care colleagues. Candidates should be experienced Registered Nurses with a working knowledge of the National Framework for NHS Continuing Healthcare and associated processes Applications from registered nurses from Black and Minority Ethnic (BME) backgrounds who meet the essential role criteria are particularly welcomed as they are currently underrepresented in senior positions and we are striving to redress this balance and achieve our WRES objective by supporting career progression and development into these roles. The post-holder will provide clinical input in the assessment and case management process for CHC as part of the wider hub function and will ensure that work programmes are planned, managed effectively and successfully delivered. Responsible for providing clinical support and facilitating assessments and reviews against the National framework within the agreed timescales. Promoting high standards of communication and liaison with patients, their representatives and all Health and Social Care representatives. To be an integral part of Birmingham Community Healthcare Foundation Trust's (BCHCFT) Adult & Specialist Rehabilitation Services. The post-holder will provide leadership in implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care for Adult & Specialist Rehabilitation Services.
