Full Job Description

Do you want to progress your career as a valued member of the Upper Lea Valley Integrated Care Team by helping us deliver outstanding community care?, You will be expected to provide assessment, diagnosis and treatment to patients with varied conditions and needs. You will also be able to offer expert advice, guidance and information to other professionals, carers and relatives and other non-professionals., Autonomous practitioner who must also work collaboratively in order to provide advanced and comprehensive assessments, reassessment, treatment and rehabilitation of complex patients with diverse physical and cognitive presentations including palliative care, wound care, acute illness, falls, frailty, post hospital care and long-term condition management in the community.
Support the operational management and day to day running of the team.
Assess patient understanding, gain valid informed consent and engage with significant others to work within a legal framework with patients who lack capacity to consent to treatment.
Ensure a high standard of clinical care for the patients under the care of HCT.
Provide spontaneous and planned specialist advice, teaching and instruction to relatives, carers and other professionals.
Be responsible for the accurate maintenance of patient records in line with HCT Policy
Develop and provide advanced clinical leadership and education to all clinicians and support staff working in the community setting
Support the induction of new staff and the supervision, education and assessment of other clinicians.
Ensure safe clinical practice and use of equipment by others through teaching, training and supervision of practice.
Take a lead role in audit, research and Quality Assurance activities in line with HCT Clinical Quality Strategy.
Identify areas for development and implement/be involved in initiatives which lead to changes in service provision which benefit our community.
Communicate effectively, sensitively and professionally with patients/carers/colleagues in a non-judgmental form that is appropriate to them and the situation including the exchange of factual information, using advanced communication techniques which respect barriers and patient's dignity, wishes and beliefs.
Build relationships across professional and organisational boundaries, using influencing and negotiating skills to coordinate with others to ensure our patients receive care that respects their wishes.
Support HCT recruitment activities of other clinicians that wish to work in the community.

You'll be making a real difference to your patients every single day, with support from the organisation and your colleagues every step of the way., You will also be able to demonstrate sound clinical reasoning and communication skills. We are also looking for an ability to work within a team and to be able to use your initiative.
Our service operates 7 days per week. Full time and part applicants are welcomed.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
