Full Job Description
We are looking for someone to come and lead our Smoking cessation Service, operating at both Harefield and Royal Brompton Hospitals to support our patients from their inpatient admission through to beyond discharge. You will need to be able to work from both venues with your time split equally between the two and a significant amount of work is undertaken virtually.
You will work clinically alongside your team as well as undertaking line management of the team and developing the service with support from the clinical lead.
Working for our organisation
In conjunction with other members of the smoking cessation multi-professional team you will be responsible for providing expert clinical practice to our wide cohort of patients including offering education in both an inpatient and outpatient setting alongside leading our smoking cessation services. Our services includes face to face and virtual interactions.
You will be required to work closely with your MDT colleagues and contribute to the development of a high-quality, evidence-based service. You will have an opportunity to help shape and promote the service, providing you with an opportunity to engage in service development and evaluation.
CPD and professional development is supported by the trust., To promote a healthy lifestyle through advice and support patients and relatives/carers in one to one and group sessions, via telephone, via video calls and face to face. Work will predominantly be within the smoking cessation arena in line with the NHS Long-Term Plan, with time spent in the CVP&R service in line with professional background.
To consolidate and develop knowledge and skills through participation in continuous professional development (CPD). The post holder is required to deliver a high standard of care, working collaboratively with the highly specialist practitioner in the development of this new service.
This role will provide clinical practice to support and facilitate high quality individualized care to patients/relatives/carers without direct supervision, in collaboration with the multidisciplinary team. Work to be undertaken will be based around smoking cessation activities with time also spent educating patients pre and post cardiac procedures, following up cardiac patients post discharge and involvement in our exercise service in line with your professional background.
To independently perform advanced assessments of patients with diverse presentations and complex physical and psychological needs and deliver an individualised and specialised programme of care. To undertake all aspects of clinical duties as an autonomous practitioner with freedom to act on own interpretation of data without direct supervision. Supervision takes the form of regular formal training, clinical reasoning sessions and peer review. Access to advice and support is available from senior team members if required. Clinical work is not routinely evaluated.
To consolidate and develop knowledge and skills through participation in continuous professional development (CPD). To use evidence based principles/current best practice to assess, plan, implement and evaluate interventions.
To undertake supervision, education and assessment of competence of team members and students as required. This would involve working with Universities to ensure standard of practice and teaching meets the standards set by degree level qualification.
To actively participate in service expansion plans which include extending appointments offered into the early evening and weekends on a rota system, as well as travelling to community venues. Whilst you will be based primarily at one of our hospital sites, due to the cross-site nature of this service, you will need to be willing to work on the other site in the case of staff absences (inter-hospital transport available).

Do you have a passion for smoking cessation and preventative healthcare? We have an exciting opportunity for someone to come and lead our smoking cessation service, as part of the wider cardiovascular prevention & rehabilitation team! Our service covers both inpatients and outpatients.
You will lead the service and with support of our multi-disciplinary smoking team ensure our patients receive a high standard of care. As patient focused registered healthcare/public health professionals, you will ideally have experience and expertise in health education, cardiovascular care and smoking cessation, although training will be given and appropriate CPD will be supported. You will also have a proven record of leadership and supporting career development of your team.
Your background may be in a wide range of NHS or health related professions (including but not confined to nursing, physiotherapy, occupational therapy, psychology, pharmacy).
