Full Job Description

The Clinical Practitioner in Long Term Conditions (CPLTC) will act as a generalist 'expert' and exercise clinical expertise in the long-term conditions (LTC's) including frailty, levels of judgement, discretion and decision making in clinical care.

· Workcloselywithwider multi-disciplinary workforcein primary, communityandsecondarycaretoensurepatientsreceiveappropriateinvestigation,interventionandtreatmentwithminimalavoidabledelays,whilstimprovingthe quality of care in the right setting.

· Workautonomouslyassessingindividualpatientneeds,initiatinginvestigationsandinitiating appropriate holistic, best evidenced based treatment with the development of apersonalisedclinicalmanagementplan,includingindependentprescribing.

· Ensure the patient is placed at the center of all decisions, reinforcing the concept ofamorestreamlinedpatientjourney,improvingthequalityofcare.

· Monitor and lead improvements to standards of care through, supervision ofpractice,clinicalaudit,evidence-basedpractice,teachingandsupportingprofessionalcolleaguesandtheprovisionofskilled professionalleadership.

· Work in partnership with care home providers, managers, primary care, patients and the wider health and social care team to maintain high quality care, prevent avoidable admissions hospital admissions and reduce length of stay.

· Use a risk stratification tool to identify patients who are vulnerable to hospital admission and clinical harm.

· Rolemodelstandardsofcareandbehaviourthroughclinicalpractice.

· ContributeandwhereappropriateleadthedevelopmentoftheMulti-Disciplinary Team (MDT) and teamobjectives forthearea ofexpertise.

· Act as role model of standards for behaviour and professions, throughcommitment to the integration of policies and procedures within role andworkplace.

· Contribute to local networks and specialty specific groups, working inpartnershipwithotherkey professionals.

· Analyse relevant national policy, within area of expertise, advising onimpact for service, leading the implementation of changes and evaluationofprocessesin partnership with the MDT.

· Maintainprofessionalregistrationandpracticethroughcontinuous professional development.

· Maintain personal standards of conduct and behaviour consistent withTrustand NMC/HPC/ other professionalbody guidelines.

· Work in accordance with relevant professional bodies Nursing Midwifery Council/ AHP Code of Professional Conduct.

· Beachampionforthecareofthepatients with LTC's including the olderperson with frailty.

· Practiceautonomouslyandself-directed.

· Work within a MDT to plan and implement high quality care.

· Demonstrate MDTworkingskillstosupportotherswithextendingtheirprofessionalroles.

· Activelyparticipateinservicedevelopmentandnewwaysofworking.

· Provide health promotion and prevention orientation, and comprehensivelyassessthepatientsforriskfactorsandearlysignsof illness.

· Havehigh-levelcondition-specificclinicalknowledgeandskillsandmaintaincompetence.

· Provide high-level assessment, diagnosis, planning and implementationprogrammes of evidence based care for the patient group, and be able tore-evaluationimplemented treatmentplansasappropriate.

· Assess and manage complications or exacerbations of LTC, this mayrequireadditional diagnosticandtreatment skills.

· Ensureinterventionsare takeninlinewith best practice guidance.

· Undertakeadvanced healthskillsappropriatetorole,ensuringclinicalcompetencies are maintained in accordance with relevant framework foradvancedpractice and scope of practice.

· Provide advice, information and support for Trust staff caring for specificclientgroups.

· Practiceasanindependentprescriberwithintheboundsofcurrentlegislation.

· Provideexpert knowledgeadviceandsupporttoclientgroups.

· Delivertechnicalskillsrequiredforinterventionforspecialistpatients.

· Identifylocal,demographic,culturalandenvironmentalfactorsthatinfluencethehealthand quality oflifeforclient groups.

· Work effectively and sensitively with relatives who are highly distressed and who may exhibit verbal or challenging behaviors.

· Contribute to the co-ordination and effective management of the admission and discharge process and referral to frailty services.
