Full Job Description
The post holders will be part of the Psych Liaison Team, formerly, RAID (Rapid Assessment Interface Discharge) Service which is a model of
psychiatric liaison serving the Newham University Hospital including the Emergency Department (and inpatient wards) embedded in the integrated
care pathways operating within the site.
As a Liaison Nurse Practitioner you will be supporting the patient journey, providing specialist mental health input and support.
You will be competent to conduct full mental health assessments and management plans for people that are referred to our team from the general wards and the Emergency Department for a variety of reasons including emotional problems, self-harm and deterioration of their mental state., Be responsible for the full mental health assessment of care needs of patients presenting with
a wide variety of clinical conditions within the Emergency Department, the acute hospital and
the wider crisis pathway. Following this, the post-holder will be responsible for development,
implementation and evaluation of programmes of care without supervision, unless otherwise
indicated.
2. Have freedom to act, particularly, but not exclusively, around discharge and follow-up in line
with the psychiatric liaison service key performance indicators on discharge. In doing so, they
will at all times, work closely with members of the psychiatric liaison team and organisations
and teams as appropriate to the needs of the patient and within the set timeframes.
3. On a secondary basis, provide a comprehensive consultation-liaison service to the other
medical teams and individual clinicians, particularly assisting them develop plans of care and
make complex decisions in cases where patients over the age of 16 years are exhibiting
complex psychosocial needs and/or challenging behaviours.
4. Take an active role in contributing to meeting the psychiatric liaison service key performance
indicators as specified in the service specification.
5. Provide independent judgement and advice on complex facts and situations, for example with
regards to gatekeeping, which can be conflicting and multi-dimensional and where expert

People presenting to the Emergency Department will often be in crisis and you should have the skills to adapt to a variety of situations, including managing a busy workload against time constraints.
You will hold a sound knowledge in the relationship between physical and mental health.
You will be required to make clear autonomous decisions based on a variety of complex presenting problems. You should possess excellent
assessment and risk management skills alongside excellent intra-personal skills including the ability to communicate with clients and all other
health care professionals, and excellent skills in time-management and maintaining appropriate clinical records.
