Band 7 Senior Complex Discharge Nurse

The successful candidate will oversee the discharge hub and ensure complex patients are discharged safely from Hospital to the appropriate care setting within a timely manner. The ideal candidate will have relevant experience in leadership and management and oversee effective assessment and discharge planning working in collaboration with both internal and external partners.
